Comprehensive Disease Management Program (CDMP)

The Comprehensive Disease Management Program (CDMP) is a web-based software application originally used to manage patients with diabetes. Designed and built-in cooperation with the Joslin Diabetes Center and multiple government health care and private industry partners it has evolved to be a general chronic disease management application capable of supporting the patient medical-centered home model. It is currently in use at multiple government, academic, private, and community health clinics around the nation. Designed as a framework, CDMP currently targets Diabetes, Chronic Kidney Disease, Congestive Heart Failure, and Hepatitis B and C.
