The Chronic Disease Management Program (CDMP) is a web-based software application used to manage patients with chronic illnesses built-in conjunction with the Joslin Diabetes Center and multiple partners. Designed around the Chronic Care Model, it is currently in use at multiple government, private, and community health clinics around the nation. Designed as a framework, CDMP currently targets diabetes and chronic kidney disease. Management of Hepatitis B and C as well as Liver Disease are in progress.
