Expanding CDMP

Estenda has initiated projects to expand CDMP to include chronic kidney disease, liver disease and hepatitis B & C.

Working with the National Kidney Foundation of Hawaii Estenda is expanding the rules, guidelines, and data presentation in the CDMP application to manage patients with Chronic Kidney Disease. The ultimate goal is to create a statewide CKD data repository of anonymized patient information to facilitate research and improve outcomes.

Along with this project, Estenda is working with Hawaii Medical Service Association (HMSA), University of Hawaii, and Hawaii Medical Center East to customize CDMP to handle Hepatitis B and C screening, monitoring, and treatment.
